Young People’s Services
It’s our aim to make transitions a positive experience for the young people we support and their families. We support young people who are approaching transition and getting ready for adult services to play a key role in choosing their future.

An Overview
Our transition services are for people aged 18 plus as they transition from living with their family or moving from children’s services or residential schools.
We work with young people, their families, and social care professionals to create bespoke pathways by exploring options such as housing, vocational opportunities, education or community activities. We’ll work together to find the right placement to meet their needs.
Young people can move into one of our residential services, bespoke supported living or a supported flat scheme.
