Our Referral Process

Finding the right support is a collaborative process. We take time to understand each person’s needs and
work closely with families and professionals to make placements are right.

Most referrals take around 90 days, depending on individual needs and funding arrangements.

Step 2

About You

We review assessments and speak with everyone involved to understand needs.

Step 3

Finding a Home

We identify a suitable home based on compatibility, support needs and availability.

Step 4

Assessment

Our team completes a detailed assessment and risk review.

Step 5

Visit the Home

You can visit the proposed home and meet the team.

Step 6

Funding

We confirm funding with the local authority or ICB and agree a start date.

Step 7

Transition Planning

We plan the move carefully, with visits to ensure a smooth transition.

Step 8

Moving In

We support move-ins and review progress after six weeks.
