Moving into adulthood can be exciting, but it can also feel uncertain, especially for young people with complex needs and the families supporting them.
Growing up doesn’t mean going it alone. We provide transition support for young adults aged 18+. We assist with the move from children’s services into adult life, offering environments where young people can build independence.
Moving on from school, college or home
Leaving residential school, specialist college or long-term support at home is a major milestone.
For many young people with learning disabilities, autism or complex needs, this transition into adult services can feel daunting. Families worry about finding the right environment where their loved one will continue to grow and develop independence.
Transition support is about creating a pathway into adulthood that feels stable and achievable.

Moving from residential schools or colleges
Helping young people continue developing independence after education or homes they've been for some time.
