We welcome referrals from anyone who might be interested in the services that we provide, whether private placements or from local authorities or housing associations.
What is the initial referral conversation about?
A member of our team will talk through your needs. This may simply be providing you with information or it may be organising for one of the team to meet with you as a next step in our referral and assessment process.
They will talk you through the options available to you and tell you about the next stage. They will also will capture key information from you or your representative over the phone. This will include the location you want to live, your social care and health needs and your wants and wishes.
This information enables us to source the right service to meet your needs.
What is the assessment process?
Following the initial discussion and gained the information we need, we will pass the details to the right person in our team for consideration.
An experienced member of our team will arrange to meet with you and your family members or social care professionals to carry out an assessment of your needs.
This lets them identify and plan the best support for you. It lets them find out more about you so they can identify and plan the best support and services in line with your bespoke needs.
What does the assessment involve?
You will lead the assessment process and it will also involve your family members and health and social care professionals.
The aim is to capture as much information as possible about you so we can find the right placement to meet your needs.
